Welcome to Tri-State Neurosurgical Associates.  We are here to help you feel comfortable during your visit to see a neurosurgical specialist.  We will help you be prepared and answer any questions regarding your upcoming appointment.

BEFORE YOUR APPOINTMENT

 

Once you are scheduled to see a Neurosurgical Associate Specialist you will receive confirmation of your appointment, directions to the office location and a medical history form in the mail.  We will contact you around two days before your appointment to confirm it, review any instructions and complete the pre-registration process.

WHAT TO BRING TO YOUR APPOINTMENT

• A list of all your medications and other supplements you take

• Your Insurance or Medicare card

• Completed Medical History Form

• Any medical records, CD's of X-Rays, MRI or CT Scans

• A Government issued photo ID, like a drivers license
